Advancing Health Care Access and Delivery Conference
Target Audience
This activity is designed for physicians, nurses, and social workers. Other learner groups such as medical students, psychologists, licensed counselors, dietitians, health professional students, educators, healthcare staff, front office personnel, schedulers, individuals who identify with the populations sharing their stories (veterans, people experiencing homelessness, and the transgender community), and community allies would also benefit from this education.
Learning Objectives
At the completion of this course, the participant should be able to:
- identify key health care gaps affecting veterans, individuals experiencing homelessness, and transgender patients, and describe how social drivers of health contribute to inequitable outcomes
- describe structural factors that impact access to health care and health outcomes for patients with complex health and social needs, including veterans, individuals experiencing homelessness, and transgender patients
- identify communication techniques that mitigate mistrust among patients who have experienced discrimination or fragmented care systems
- interpret patient narratives with humility, including recognizing how stigma, trauma, and past negative encounters may shape patient engagement
