At the conclusion of this session, the participant should be able to:
1.Recognize the CKD detection gap.
•Identify barriers to early CKD diagnosis and albuminuria screening in routine practice
2.Evaluate Clinical Decision Support (CDS) tools for CKD identification.
•Compare two clinical decision support tools used to improve albuminuria screening rates at Geisinger
•Describe the design and impact of EHR autoloading of CKD diagnoses to the patient problem list at Geisinger
3.Discuss different tools for CKM syndrome management.
•Outline future applications of CDS in combination with other tools for better management of CKM syndrome
