At the conclusion of this session, the participant should be able to: 

  • describe the rationale for and history of developing a standardized clinical pharmacy patient care process.  
  • define comprehensive medication management (CMM), explaining key differences between CMM and other practice models (e.g. Disease State Management, Medication Therapy Management). 
  • describe evidence for the impact of CMM on the Quadruple Aim.
  • discuss practical application of CMM in ambulatory care clinical pharmacy practice.
