Series Overview
To learn how to start this very important conversation about what matters most to your patients and how to integrate person centered conversations into your everyday practice. It’s not about what is the matter with your patient but rather what is matters most to them. Every learner should be equipped with the communication skills that assist in having quality discussions with patients/families about future care.
Target Audience
This activity is designed for clinicians working with the adult population, including - nurses (case managers) in any setting, social workers, advance practitioners, respiratory therapist and physicians. Other learner groups such as Medical Assistants, Community Health associates, Patient Access Representatives, and Chaplains would also benefit from this education.
